Notice: As set forth in Executive Order 311 signed by Governor Murphy on November 16, 2022, all legal name changes on file with the Division of Revenue and Enterprise Services (filed in 1948 or later) are now classified as confidential. (See ).
The Division may grant exceptions to the confidentiality provision. If you are the individual who obtained a name change judgment, an authorized representative of the individual, or a public official with a valid governmental reason to access the judgment, then you may submit a Good Cause Exception form , along with the required documentation.
Court-ordered name changes must be filed with the Division within 45 business days of the name change. The original certified copy of the court ordered decree with raised seal must be submitted with a filing fee of $50.00. The original decree will be returned to you. Should you require a certified copy of the filing, an additional fee of $25.00 per copy is required.
