Sample Digital Claims

State Agency Claim Login Page

State Agency Representatives that wish to file a claim on behalf of their State Department, agency, Division, or institution, must first email the Division of Risk Management for permission and access to the State Agency Claim Login Page and portal. 

Upon receiving your request, the Division of Risk Management will send you an email invite so that you can access your secure portal via the ÐÇ¿ÕÎÞÏÞ´«Ã½app’s network, and file your digital claim form.

Please send you request to the following email address: State.NJClaims@treas.nj.gov.  Kindly ensure to include the following:

  1. Subject section:  Request to file a new State Agency Claim
  2. Body of email:
  •  Specify the type of claim you wish to file (Comprehensive, Subrogation, or Claim for Damage to State Property (State Property Insurance Claim)
  • Date of Accident
  • provide additional relevant details you deem necessary.

Note: You can email IT Customer Service and/or log in to a live Teams Meeting with a tech representative on Mon/Wed/Fri from 11 a.m. - 12 p.m. or 3 p.m. - 4 p.m.
